Accident investigation
Training Objectives
Accidents investigation, analysis and reporting should be part of the safety management system of any company. Only then, one can minimize the recurrence frequency. This course provides guidance on how to use the databases; how to investigate accidents and incidents by using some techniques; how to identify the root causes and underlying causes; and how to record the results.
Training Subjects
• Introduction: safety management systems; performance indicators; accidents; incidents; legal requirements; lessons from past accidents; databases
• Accidents in the Brazilian process industry: summary of types and number of accidents and incidents; trends and comparisons with other industries and other countries; specific examples of some types of plants, including the latest statistics; cost of accidents; does the industrial reality meet the ideal models?
• Incident investigation techniques: root cause analysis, fault tree analysis, causal factors diagram, multiple events sequencing, Barrier analysis, modifications analysis and “TapRoot” system
• Accident databases: use of databases in the project early stages; MHIDAS, FACTS, IChemE, Fire Department, etc.; corporate memory; inherently safe projects
• Legal aspects: involved public agencies, access of third parties to the company documents; companies rights; insurance companies; statements; subsequent legal actions; court processes
• Use of specialized external services: advantages and disadvantages of independence; the role of a specialist; stakeholders representatives
• Exercise: short syndicate work; description of an accident or incident; groups defining actions to be taken
